Might it be an idea for someone with a copy, or the author, to investigate digital technology. Either scanning the book to send out as PDF file or burning it to CD & selling CD's.

I second that idea Louisa! I would love to read it, but £500 for a book?? You've got to be kidding!!?? If a reprint is difficult/costly perhaps an electronic version would be a possibility?
